12. Forms, Registration & Data
12.1 Standard Forms (VONDA)
Directors ensure that: A standard camper registration form is used for all camps. Family Camp / Reunion forms reflect flexible attendance patterns. Medical and consent forms meet current policy and legal requirements.

12.2 Data & Evaluation
Post-camp, we collect:
Camper feedback (age-appropriate). Parent feedback (Vonda). Staff feedback on workload, support, and burnout.
Track basic metrics:
Number of campers and staff by age group. Returning vs. first-time campers. Notable incidents and learnings.
Feed this back into planning for future camps.

Camper Days Attendance Report
Camp programs These numbers capture full or part day attendance of individuals. Children Youth Adults Adults Adults 56+ Total Ages Under 13 13-18 19-30 31-55 56+ CAMP NAME
In order to accurately track camp participation, your assistance is needed in reporting the attendance at your camp. Please ensure your registration form tracks ages in this manner. The camp director may designate this responsibility to the business manager.
"Please complete this report for your camp and send to camping@communityofchrist.ca within 2 weeks of camp closing. It should also be copied to the Campground Board Chairperson responsible for reporting camper days. This report includes numbers for all who have registred for the full or part week of a camp. It also includes those attending the setup day prior to camp start. Some reunions waive the registration fee for reunion day visitors. As this report is used for insurance purposes, please ensure day visitors at your camp/ reunion sign in on an attendance sheet. Include those numbers in this report. Keep attendance sheet with your other reports."
